**Role summary**:
To provide compassionate and professional palliative nursing care to patients and their families as a part of the inter-disciplinary care team. Inform stakeholders of palliative care arrangements within the community-based service platform. Establish effective care pathways and linkage throughout the care continuum. Ensure effective management of palliative care clients in home and community-based settings.

**Task summary**:

- Patient and family care
- Referral pathways
- Support by the Local PHC Facilities and referring hospitals
- Clinical governance/oversight
- Monitoring, evaluation, reporting, and record keeping
- Inter-disciplinary team
- Professional responsibilities
- Administrative responsibilities
- Medical aid clients
- Management of patients and staff after exposure or testing positive for covid-19**Essential**:

- CURRENT REGISTRATION WITH SANC AND COMPLETED COMMUNITY SERVICE
- PALLIATIVE CARE TRAINING
- VALID DRIVERS LICENCE
- **MUST HAVE OWN VEHICLE**
- Good communication skills, written and verbal.
- Strong organization skills, ability to multi-task.
- Well-developed computer skills (word, excel).
- Strong interpersonal skills.
