- We are looking for an energetic and dynamic _**_Outreach Team Leader: Professional Nurse_**_ to lead the Home-Based Carers, CDU, CDL, Rehab Adherence, CDU Staff._
**Key Duties and Responsibilities**:
**Provide Clinical Care to Patients**
- Provide clinical care to patients.
- Assessment of all clients upon admission to the NPO (before clients are allocated to specific CHW) per hospital/institution's referral letter.
- Ensure the implementation of client bedside records.
- Attend personally to complicated dressings.
- Use the prescribed assessment tool to categorize each client.
- Develop an appropriate care plan for each client as Dr’s orders.
- Provide necessary support and facilitate in-service-training to relatives/primary caregivers in the home environment.
- Do regular follow-ups and spot checks on clients.
- Set up daily work schedules for the Community Health Worker (CHWs)
**Recruitment, Selection & Training**
- Conduct regular orientation sessions with health workers.
- Assess & identify training needs of all carers.
- Ensure all health workers have completed the standardized training curriculum presented by PGWC.
- Present/facilitate on-going, in-service training to Community Health workers.
- Facilitate and support the EPWP training.
- Promote team building exercises for health workers.
**Administration**
- Establish a set of records and ensure that they are up to date.
- Supervise the administration of all matters relating to Home Based Care and Integrated areas.
- Ensure that the necessary reports are written and submitted.
- Ensure that all CHW givers work within their scope of practice.
- Follow all policies and procedures as required by the organization and ensure that all work targets are met.
- Identify resources; equipment and supplies that are needed and follow required ordering process to access these from Technical Assistant.
**Report/ Meetings**
- Establish a set of records and ensure that they are up to date.
- Participate in meetings as required as well as attending training courses and workshops related to the nature of the work done.
- Check CHW workers time sheets, verify reports, and submit monthly stats and quarterly reports timorously to sub-district Technical Assistant.
- Submit same reports and stats to NPO Management.
- Attend Regional HBC quarterly meeting, bi-monthly HBC Forum meetings and bi monthly Rehab Forum meetings.
- Conduct weekly meetings between CHW's and Coordinators.
**Monitoring and Evaluation**
- Establish a set of records and ensure that they are up to date.
- Participate in meetings as required as well as attending training courses and workshops related to the nature of the work done.
- Check CHW workers time sheets, verify reports, and submit monthly stats and quarterly reports timorously to sub-district Technical Assistant.
- Submit same reports and stats to NPO Management.

- Continuous assessment and evaluation of CHW's competencies.
- Regular visits and spot checks.
- Monitor and evaluate CHW's service delivery.
- Monitor and evaluate client satisfaction and client progress.
- Update client categories on a monthly basis.
- Monitor and ensure Care Plan compliance and reporting.
- Refer clients back to CHC's (timeously) if no progress or condition deteriorates.
**Networking and Advocacy**
- Local community clinics, Community Health Centres and day Hospitals.
- Liaise with all other organizations at grassroots level as necessary.
- Liaise with Referring institutions.
- Identify areas that need special support and institute the necessary help.
- Develop awareness within the community of relevant HIV/AIDS TB and other related issues and assist staff to encourage community members to take responsibility for their health.
