Overview Social Worker at Nurture Stellenbosch – Western Cape. Reporting to the Therapy Manager, the Social Worker is an essential member of the interdisciplinary rehabilitation team. Responsibilities Perform comprehensive psychosocial assessments and craft individualized care plans. Provide counselling, crisis intervention, and ongoing emotional support to patients and families. Facilitate family meetings, discharge planning, and smooth community reintegration. Advocate for resources, benefits, and funding to optimise long-term outcomes. Collaborate daily with the interdisciplinary team to deliver seamless, evidence-based care. Maintain accurate, confidential documentation compliant with SACSSP standards and contribute to quality-improvement initiatives. Qualifications & Experience BA Degree in Social Work. Current SACSSP membership & valid malpractice insurance. 2-3 years' experience in a private hospital or rehabilitation setting (advantageous). Strong communication skills: clear, compassionate, and culturally sensitive. Effective collaboration: thrives in a multidisciplinary team; builds trust quickly. Analysis & decision-making: sound clinical judgement and ethical practice. Safety and quality: champions best practice and patient dignity at all times. Initiative-taking: proactive, solution-oriented, and adaptable. Flexibility: comfortable with evolving priorities and diverse caseloads. Time management & organization: balances caseload and admin efficiently. Compensation Minimum Salary: R (cost to company, including benefits).
