SUMMARY Care Manager - Cape Town, Western Cape POSITION INFO Job Title: Care Manager Employment Type: Full-time, permanent Location: Panorama, Cape Town, Western Cape Summary of Job To set and maintain the standards of general care and nursing care; to monitor and control the delivery of care, in accordance with company standards. To determine the changing needs of residents and ensure the design of appropriate interventions. To ensure compliance with the South African Nursing council requirements at all times. Plans, organises, directs controls and coordinates nursing programmes and care to residents. Key Responsibilites 1. New Resident Ensure that new residents are timeously pre-assessed and all relevant documentation is available and / or completed. 2. Care Plans and Assessments Ensure that care plans are in place for all residents. Ensure that care plans are relevant, updated and applicable to the changing needs of the individual. Care plan meetings are held on a regular basis (minimum of once a month). 3. Care Services Ensure accuracy of homecare costing to the clients. Ensure changes in homecare services are communicated to the Care team. Care plan and checklists updated. Ensure that all services are co-ordinated and implemented. Ensure that any changes in homecare services are communicated with relevant family and costs are discussed. Ensure that the homecare services costing is accurate and submitted to the Branch Accounts Administrator on time. Ensure that all consumable charges, e.g. wound dressings / incontinence products, etc. is accurate and submitted to the Branch Accounts Administrator on time. 4. Management of Unit Ensure a participative management style exist by consulting the team on care related issues. The rosters are signed off and placed on the notice boards. Spot checks should be conducted by Supervisors, Nurses and Care manager, on day-to-day care delivery. Proof of spot checks should be evident on record. Ensure the availability of clinic services. Residents Questionnaire for Care. Care Audit and outstanding matters. Ensure that the Care strategy is successfully implemented. Ensure that all supervisors, EN''s, RN''s understand and comply with the performance management process. Ensure that all performance appraisal issues are dealt with timeously. Recruitment : Appointment of staff. Probation Evaluation. Ensure that complaints are thoroughly investigated by assigned / designated person (RN). Communicate and give timeous feedback to residents / family / friends regarding any complaints lodged. Disciplinary action. Ensure that all CCTV cameras are operable and in working condition. Administartion / Reporting. 5. Training and Development Maintaining of training registers. Ensure that health talks / seminars / workshops are planned, schedule and marketed. 6. Meetings Prepare and attend for all management committee meetings. Arrange weekly Care meetings. Arrange ADL / Care Plan meetings. 7. Promote and Maintain Safety Environment is safe Ensure that all standard emergency incident procedures are followed and documented. 8. Maintaining of relationships with residents and families of new residents Introduce yourself to the new resident within one week of welcoming pack received. 9. Budget Submit bugets minthly / annually. 10. Health & Safety Health & Safety standards maintained 11.
