About the Role As a Clinical Nurse Specialist Grade 2 you’ll have a high level of autonomy, managing your own patient list and providing clinical care in the home environment, while collaborating with doctors and the broader multidisciplinary team (MDT) to ensure the best possible outcomes for patients and carers. Independently manage and prioritise your patient caseload Deliver complex clinical care and problem‑solve in dynamic community settings Coordinate seamless transitions for patients within Supportive and Palliative Care services Advocate for patients and families, providing education, guidance, and emotional support Maintain accurate documentation and evidence‑based care plans Be supported by the Nurse Unit Manager and the Nurse Manager of Lower Hunter Community Health, ensuring you have guidance and mentorship as you settle into the role Receive a comprehensive orientation program and access to My Health Learning Who You Are We’re seeking a skilled, compassionate, and motivated clinician who thrives in a community‑based role that blends independence with teamwork.
